肝PEComaの1切除例

要旨
症例は45歳，男性．人間ドッグの超音波検査で肝腫瘤を指摘され当科を受診した．血液検査所見で肝炎ウイルスは陰性で肝胆道系酵素，腫瘍マーカーも正常範囲内であった．画像検査所見で肝S5に37 mm大の類円形腫瘍を認め，造影パターンなどからは非典型的であるが肝細胞癌などの悪性腫瘍の可能性を否定できず，S5亜区域切除を施行した．病理，免疫組織学的検査所見で，腫瘍は平滑筋細胞が密に増殖し，human melanoma black-45, smooth muscle actinなどが陽性で，肝PEComaの診断となった．Malignant potentialのある腫瘍であり，肝腫瘍の診断，治療の際には念頭に置く必要がある．
