【ポイント】
◆胃全摘術後の再建はRoux-en-Y法が最も多く行われているが，簡便性や術者の経験による影響が大きい．
◆現在，パウチ作製による空腸間置法やY脚部にパウチを作製する術式が試みられており，術後QOLの向上が期待される．
◆今後は，急速に普及した腹腔鏡手術も念頭に，エビデンスレベルの高い大規模なRCTを行う必要がある．
