【ポイント】
◆幽門側胃切除術はおもに胃癌など胃の悪性腫瘍に対する手術で，胃の中部から下部にある腫瘍が対象となる．胃癌治療ガイドラインでは幽門側胃を2/3以上切除するのが基本となっている．
◆胃切除後症候群とは胃切除術後に生じる様々な症状の総称で，小胃症状，ダンピング症候群，便通異常，逆流症状といった消化器系のもののみならず，慢性的な消化吸収障害から発生する栄養不良，貧血，骨代謝障害なども含まれる．
◆二重空腸囊間置再建法（JPI法）は幽門側胃切除術後再建法の一つで，二重空腸パウチと順蠕動性導管からなる有茎腸管を，残胃と十二指腸の間に間置するものである．パウチが小胃症状を改善し，順蠕動性導管がダンピング症状と十二指腸液の逆流を防止する．
