初回治療17年後に発症した頭蓋内血管周皮細胞腫の膵転移の1切除例

要旨
症例は59歳，男性．42歳時に頭蓋内血管周皮細胞腫（HPC）の切除を施行した．その17年後に腹部不快感を主訴に前医を受診した．画像検査にて膵頭部，左腎，両肺に多発する腫瘍を認め，組織診断の結果，頭蓋内HPCの多臓器転移と診断した．膵頭部腫瘍が強い腹部不快感の原因であり，今後の腫瘍増大による出血や消化管通過障害出現の可能性を考慮し，膵頭十二指腸切除術を施行した．腹部不快感は軽快し，食事摂取量が増加した．術後1年6か月間，増悪なく経過観察中である．頭蓋内HPCの膵転移は稀少であり，貴重な症例と考え報告する．
