【ポイント】
◆乳がんの治療においては，まず生物学的特性（バイオロジー）に基づいた分類であるintrinsic subtype（サブタイプ）に分けて治療方針を立てる．
◆サブタイプごとに予後や薬物療法の感受性は異なる．そのため初期治療の組み立てからサブタイプを考慮する必要がある．
◆neoadjuvant therapy（術前薬物療法）の主な目的は，①腫瘍縮小効果によってより侵襲の少ない局所治療を可能にすること，②抗腫瘍効果を直接みることによって次の治療につなげられること，③予後（再発率，全生存率）の予測を可能にすること，などである．
◆病理学的完全奏効（pCR）をエンドポイントとした臨床試験により新薬の効果が速やかにわかるため，FDAでは迅速承認としての役割を認めている．
◆将来的な可能性として，①術前薬物療法を行い十分な効果のみられなかった症例に対し，追加の治療によって予後を改善させること，②術前薬物療法を行い十分な効果がみられた症例に対し，追加の治療を省略できること，③pCRの場合，適切な画像診断によって非手術とすること，などが挙げられる．
