レネック被膜とは何か？—安全確実な肝切除の定型化のために

はじめに
　肝切除は肝臓がんに対する唯一の根治的治療法として広く普及しているが，実際には現在でも危険性の高い手術である．肝切除を安全確実に実施するためには，流入および流出血行の適切なコントロールが不可欠であり，高崎らが提唱した「肝外グリソン鞘一括先行処理による解剖学的切除（Glissonean pedicle transection method）」^1,2）は理想的で，腫瘍学的にも優れている．しかし，その手技はいまだ定型化されているとは言いがたい．「肝外グリソン鞘一括先行処理」の手技を定型化するためには，レネック被膜の存在とその外科的意義を理解することがきわめて重要であり，それによりあらゆる肝切除の安全性と根治性が確立されるものと期待される．本稿では，レネック被膜の概念と，それに基づいた肝外グリソン鞘確保の手技を中心に解説する．
