経十二指腸的乳頭部切除の適応と限界

【ポイント】
◆経十二指腸乳頭部切除は局所切除術であり，十二指腸乳頭部の良性腫瘍，もしくはリンパ節転移を伴わない粘膜癌が適応である．
◆Oddi括約筋まで癌が浸潤していてもtub1かつly0，v0，EM（−）であれば治癒切除になりうる．
◆姑息的治療として進行乳頭部癌に対し経十二指腸乳頭部切除を行っても，根治性の面からの縮小手術にはなりえない．
