POINT
■ウィーニング開始の絶対条件は,人工呼吸管理の原因となった病態の改善である.
■ウィーニングの遅れは人工呼吸管理の長期化を招きやすいため,人工呼吸管理を始めたその瞬間から,常にウィーニングの可能性を念頭に置いた管理を行う.
■人工呼吸中の鎮静は必要最小限にとどめる.持続鎮静中でも1日1回は患者を覚醒させてウィーニングの可否を判断する.
■ウィーニング可能と判断されれば,できる限り速やかに自発呼吸トライアル(SBT)に移行する.
