敗血症性ショックの治療

POINT
■敗血症性ショックの治療の第一段階は，輸液療法である．初期輸液量は30 mL/kgを目安とし，心臓超音波検査での評価や輸液反応性の評価を行いつつ輸液を行う．
■十分な輸液療法を施行してもショック状態が継続する場合は，血管収縮薬の投与を行う．ノルアドレナリン（初期量目安；0.05 μg/kg/分）を第一選択とし，それでも血管拡張型ショックが改善しない場合にはバソプレシンの投与を考慮する．
■敗血症性ショック患者の20〜40%には，敗血症性心筋障害を合併する．心臓超音波検査で心機能を評価し，心機能低下症例にはドブタミンなどの強心薬の投与を考慮する．
