大腸穿孔

POINT
■大腸穿孔は，糞便性腹膜炎から敗血症性ショックや多臓器不全を併発する，腹部救急疾患の中でも致死的な予後不良疾患の一つである．救命が最も重要であり，迅速な原因の解明とその病態に合わせた集学的治療が必要である．
■大腸穿孔の原因は多岐にわたるため，詳細な病歴と患者背景の把握が原因解明と治療方針の決定の一助となる．
■原因検索はCTをはじめとする画像所見に依存しやすいが，患者の表情やバイタル所見，腹部所見などにて下部消化管穿孔を否定できない場合，開腹手術による腹腔内の検索を躊躇するべきではない．
■大腸癌穿孔の場合は，救命に加えて根治性が求められる．
■本稿では，大腸穿孔の診断と治療の概要，大腸癌穿孔，医原性大腸穿孔を中心に述べる．憩室穿孔をはじめとした良性疾患による穿孔は別稿を参照されたい．
