胆囊炎症例におけるラパコレ—急性胆囊炎症例に対するラパコレ—早期LCと待機的LC

はじめに
　腹腔鏡下胆囊摘出術（以下LC）は1990年頃より革新的な低侵襲手術として急速に拡散し，近年，あらゆる腹腔内臓器で主流になりつつある内視鏡外科手術の礎である^1）．LCは2016年に改訂された「胆石症診療ガイドライン」にあるように，有症性胆囊結石症の標準術式であり，日本では約80%の症例がLCで治療されている^2,3）．一方，2000年以降，急性胆囊炎症例においてもLCの安全性と有効性が担保されることが明らかとなり，2013年に改訂出版された急性胆管炎・胆囊炎診療ガイドラインであるTokyo Guideline 2013（以下TG13）では，早期LCと待機的LCがそれぞれ重要な役割を担っている^4〜6）．本稿では，TG13に示されている急性胆囊炎治療におけるLCの位置付けと，適切な手術時期の選択方法に加え，実際の急性胆囊炎症例での注意すべき術中合併症について概説する．
