化学療法施行後に切除しえた肝門部リンパ節転移を伴う結腸癌肝転移の症例

要旨
症例は68歳，男性．盲腸癌・S状結腸癌・同時性肝転移に対して，回盲部切除・S状結腸切除術を施行した．その後，肝門部リンパ節転移が認められたため肝切除は施行せず，XELIRI＋ベバシズマブ療法を施行した．12クール施行後，肝門部リンパ節転移は陽性であったが，新規病変は認めず，肝外側区域切除術・肝門部リンパ節郭清術を施行し，根治切除術を施行しえた．術後10か月経過するが再発は認められていない．肝門部リンパ節転移を伴った大腸癌の肝転移は予後不良とされており，肝切除の適応外とされてきたが，近年の化学療法の進歩により本症例は肝門部リンパ節転移病変以外の新規病変は認められず，conversion therapyに移行することができたので，文献的考察を加えて報告する．
