【ポイント】
◆小型肺癌の手術例増加に伴い，肺葉切除術に対する縮小手術の妥当性を検証する臨床試験が進行している．
◆縦隔リンパ節郭清による予後改善効果は明らかでないが，少なくとも正確な病期決定に必要である．
◆縦隔リンパ節転移陽性例は，その程度により多様な予後を有する集団であり，症例ごとに適した集学的治療が行われている．
