鼠径ヘルニア術後にmesh plugが膀胱内に迷入した1例

要旨
症例は78歳，男性．2008年4月，当院で右内鼠径ヘルニアに対しmesh plug法にて手術を施行した．2014年10月に排尿時違和感，頻尿を認め，当院を受診した．腹部CTおよび膀胱鏡検査にてplugの膀胱内への迷入と診断し，手術を施行した．腹膜外アプローチにてplugに到達し，plugと一部膀胱を合併切除し，膀胱を縫合した．後壁補強を要しないほど瘢痕化が強かった．術後3か月現在，再発を認めていない．膀胱にplugが迷入する例は稀である．人工物を腹膜前腔に挿入する手術では，人工物が膀胱に迷入する可能性を認識し，挿入する場合には人工物が移動しないように十分な固定をすることが肝要であると考えられた．
