【ポイント】
◆胆管空腸吻合後の術後胆管狭窄は，繰り返す胆管炎などにより著しくQOLを損なう病態である．
◆胆管空腸吻合部狭窄の診断は，胆管炎などの臨床症状のほか，MDCTやMRCPなどによる画像診断により狭窄部位の同定を行うとともに，良悪性の鑑別を行うことが重要である．
◆胆管空腸吻合部狭窄の治療は，ダブルバルーン内視鏡の普及に伴い，内視鏡的治療が主流となりつつある．長期の胆管ステントチューブの留置により再狭窄を予防する．
◆内視鏡的治療や経皮経肝的治療が困難な症例，無効な症例には，胆管空腸吻合部切除および胆管空腸再吻合による外科治療を考慮する．
