【ポイント】
◆十二指腸と結腸の炎症性癒着なのか，腫瘍による浸潤なのかを術前診断することは困難な場合が多い．
◆リンパ節転移陽性症例，低分化癌や粘液癌症例，十二指腸粘膜面に腫瘍が露出する浸潤癌に対しては膵頭十二指腸切除術の併施が必要である．
◆膵頭十二指腸切除後は術後補助化学療法の導入が困難なことが多く，術前化学療法は今後の期待しうる選択肢の一つと考える．
