結腸を用いた再建術のコツと要点

【ポイント】
◆適切な層での結腸の授動．浅すぎれば結腸の血管系を損傷する一方，深すぎると余分な出血や尿管損傷の原因となる．
◆十分な血管の観察と適切な位置での血管処理．いつも同じではない．臨機応変な対応が必要となる．
◆十分な広さの胸骨後トンネルの作製と，胸腔内への脱出やトンネル内での再建臓器のたるみなどを起こさないよう注意する．
◆多少の長さ不足は十分な腸間膜根部の授動で補える．症例によっては回結腸動脈本幹を処理しない回結腸再建も可能である．
