ロボット支援腹腔鏡下前立腺全摘術後に発症した鼠径ヘルニアの臨床的特徴

要旨
目的：ロボット支援腹腔鏡下前立腺全摘術（RALP）後に発症した鼠径ヘルニアの特徴と適切な手術術式について検討した．方法：RALPが施行された187例のうち，鼠径ヘルニアは15例（8%）に発症した．成人男性鼠径ヘルニア220例242病変をRALPの既往のある群（既往群：15例20病変）と既往のない群（対照群：205例222病変）に分けて比較・検討した．結果：両側発症は対照群17例（8%）に対し，既往群5例（33%）と有意に多かった．右側発症は対照群108例（57%）に対し，既往群9例（90%）と有意に多かった．既往群では全20病変が外鼠径ヘルニアで，全例にmesh plug法を施行し，再発は認めなかった．結語：RALP後には外鼠径ヘルニアが両側または右側に発症しやすく，その術式としてmesh plug法は妥当と考えられる．
