三領域郭清を伴う食道癌切除術後に高度な気管虚血を呈した2例

要旨
食道癌手術の合併症の1つとして，気管・気管支虚血がある．頻度は高くないが，いったん発症すると治療は困難であり，重篤な経過をたどる場合もある．われわれは食道切除再建術後に気管虚血を呈した2症例を経験したので報告する．症例1：70代男性，食道切除再建術後4日目に気管虚血を発症した．保存的に軽快したが右上葉気管支の狭窄を認めた．症例2：60代男性，食道切除再建術後4日目に気管虚血を発症した．右中葉気管支に虚血を認めたが保存的に軽快した．気管虚血には気管血流温存による予防が最も重要である．左右気管支動脈の温存により気管血流の維持に努め，リスクが高い症例に関しては二期的な頸部郭清も考慮すべきであると考えられる．
