乳房に発生した多形型脂肪肉腫の1例

要旨
症例は52歳，女性．2年前より右乳房の腫瘤を自覚していたが放置していた．来院時，右乳房全体を占める18×13 cmの分葉状の弾性硬の腫瘤を認め，皮膚には潰瘍が多発していた．生検で悪性腫瘍が示唆されたが，CTでは遠隔転移を示す所見を認めなかった．以上より，右乳房に限局した巨大悪性腫瘍の診断で右乳房切除術，分層皮膚移植術を施行した．病理組織学的には紡錐形細胞，多核で多形性に富む大型異型細胞，多空胞状の脂肪芽細胞が混在し増生していた．免疫染色ではp16およびCDK4が一部陽性で，FISHによるMDM2遺伝子の増幅を認めなかったため多形型脂肪肉腫と診断された．術後5か月で肝転移が出現したが，2年経過した現在，担癌生存中である．
