高度進行消化器癌の治療戦略立案時のチーム医療における外科医の役割

【ポイント】
◆高度進行消化器癌では，診断時点で根治切除不能と判断されることが多く，初回治療として非手術的治療法が選択されることも多い．
◆治療戦略立案においては，多職種の医療スタッフが高い専門性を活かして情報を共有して互いに連携・補完し合い，最善の医療を提供することが目的となる．
◆多職種連携のチーム医療では，外科医，内科医，放射線科医，病理医，看護師，薬剤師，栄養士などが議論を尽くす必要がある．治療前の正確な画像診断，集学的治療における化学療法のレジメンや放射線療法の併用の判断が必要となる．
◆外科医は，集学的治療の全経過を通じて外科的介入の機会を逃さず，積極的に患者の利益を最大化することに努める必要がある．本稿では，非治癒切除・姑息手術（バイパスなど），減量手術などについて外科医の役割を述べる．
