【ポイント】
◆食道癌cT4症例におけるCRTから切除へのconversion therapyにおいては，組織学的効果判定およびR0切除の可否予測が不可欠である．今回，高精度の形態学的・質的効果判定法として，造影MD-CT画像再構築による組織評価を開発，応用した．
◆上記方法でのdown staging判定率は，37.9%であり，切除全例にR0判定を得た．
◆CRTからのconversion therapy（切除）において長期の良好な予後を得るためには，40 Gy時点でのdown staging判定と切除が必要と考えられた．
