【ポイント】
◆初診時に切除不能と判断された症例を切除可能に変換（conversion）できれば，胆道癌全体の治療成績の向上につながる可能性がある．
◆切除不能胆道癌に対するファーストラインの化学療法はゲムシタビン＋シスプラチン併用療法（GC療法）であるが，化学療法のみでの長期生存は望めない．
◆conversion surgery後の長期生存例が存在することから，化学療法を施行しながら切除の可能性を常に検討する姿勢が必要である．
