超音波内視鏡ガイド下インターベンションとは
　コンベックス型の超音波内視鏡を用いて，超音波内視鏡ガイド下に目的とする管腔・液体貯留腔・臓器にアプローチする手技である．アプローチ後にはドレナージチューブの留置などの治療手技を行う．管腔や液体貯留腔にアプローチする手技では，主にはドレナージであるが，ランデブー手技，結石治療，necrosectomy，狭窄拡張などの治療手技があり，経皮経肝胆道ドレナージ（PTBD）と同様に瘻孔形成後に治療を行う場合もある．実質臓器へのアプローチでは，エタノール，ラジオ波，マイクロ波などによる腫瘍のablationなどが試みられているが，本邦での報告はまだ少ない．injection therapyによる治療では，囊胞性腫瘍ではエタノールやパクリタキセルなどによる腫瘍のablation，神経叢・神経節ブロックでは薬液注入，本邦では悪性腹水に対する樹状細胞の注入療法などが試みられている．侵襲的な検査という意味では，穿刺針を通して種々のミニチュアデバイスを挿入して検査を行う，through the needle examinationが行われるようになっている．針を刺すという行為を通して種々の臓器にアプローチし，様々な検査・治療手技を行うことが可能になり，そのバリエーションは増加しつつある．本稿では，それぞれの手技の現状について解説する．
