NCDにおけるQuality Control

はじめに
　わが国における外科系医療の現状を把握するため，日本外科学会，日本消化器外科学会をはじめとして専門医制度をもつ10の外科系諸学会が協力して「一般社団法人National Clinical Database」（以下，NCD）を2010年に立ち上げて早6年が過ぎた．専門医制度と連動した手術症例の入力は2011年に開始され，初年度は1,172,579例が登録された．わが国で一般外科医が行っている手術の95%以上をカバーするとされ，2015年の登録数は1,717,186例であった．2015年までの5年間で700万件を越える手術情報が集積されており，巨大なデータベースが構築されている．登録施設も増え続け，2017年5月時点で4,967施設が参加している．忙しい日常診療の合間に，NCDから認証を受けた外科医やデータマネージャーにより，インターネットを介して中央のデータサーバーにデータ登録が行われる．NCDデータの品質を保ち，精度の高い情報を発信し続けるため，日本消化器外科学会ではデータベース委員会を設置し，フィードバック機能やAnnual Reportの公開などに取り組んでいる．本稿では，NCDデータのQuality Control（品質管理）の重要性を解説し，現在の取り組みを紹介する．
