腸重積を契機に診断し高圧酸素療法を含む保存的治療が奏効した腸管囊腫様気腫症の1例

要旨
腸管囊腫様気腫症（PCI）は腸管壁に気腫が生じる疾患である．PCIが原因で腸重積症をきたした症例の報告は散見されるが，保存的治療のみで治癒した症例の報告は少ない．今回，腸重積を契機に診断したPCIで保存的治療のみで治癒した1例を経験したので報告する．症例は17歳，女性．右下腹部痛を主訴に救急外来を受診した．右下腹部に腫瘤を触知した．腹部CT検査で腸重積症と診断し，注腸にて整復した．CTでは腸管壁に沿って多発する気腫性変化を認めた．大腸内視鏡検査では，盲腸から上行結腸に多発する粘膜下腫瘍様隆起を認め，PCIと診断した．高圧酸素療法を含む保存的治療で症状は消失し，第10病日に退院した．発症後18か月で施行したCTで，腸管壁の気腫は消失し，治癒したと考えられた．
