【ポイント】
◆門脈浸潤を伴う遠位胆管癌は外科切除のみでは長期生存は望めず，borderline resectableとして術前・術後の補助療法を組み合わせた集学的治療で予後の改善を計る必要がある．
◆遠位胆管癌においてR0切除をめざして胆管断端陰性を追求するという治療戦略は妥当であるが，高齢者，高リスクの患者，進行癌患者などに対しては，胆管断端の浸潤癌陰性を追求するが上皮内癌の有無にはこだわらないという柔軟な戦略も許容される．
◆リンパ節転移は強力な予後不良因子であり，術後の病理組織学的検査でリンパ節転移が陽性（特に4個以上）の症例は，術後補助化学療法の良い適応である．
