【ポイント】
◆胆囊癌の予後不良因子としては，肝十二指腸間膜浸潤やリンパ節転移など，広く受け入れられている因子があるが，エビデンスとして確立していない．
◆胆囊癌の予後は腫瘍浸潤（pT因子）でも進行度（pStage）でも二極化の傾向がみられる．予後不良の進行胆囊癌に関する診断から手術適応・術式選択にあたり，様々な意見・方針が混在し，標準化が進んでいない．
◆胆囊癌は術前診断が困難で，これまで切除後の病理結果を基にした予後や術式の妥当性の検討が主であった．診断時情報からの適切な治療戦略の確立に向け，前向き観察臨床試験が開始されている．
