【ポイント】
◆重症患者の血糖管理は現在のところ，持続インスリン静注による中等度TGC（tight glycemic control，目標血糖値110〜150 mg/dL）が推奨されている．
◆糖尿病の既往がある重症患者の血糖管理について現時点で明確な回答はないが，目標血糖値の上限を200 mg/dL程度まで緩和したTGCを行うことが一般的である．
◆待機手術の場合は，術前の血糖コントロール（血糖値≦180 mg/dLないしHbA1c＜7.0%）が重要である．
◆術後の血糖コントロールの目標値は，術前のコントロールの具合をみて臨機応変に設定する．術後動態が安定するまでは，インスリンを用いて管理する．
