主乳頭近傍の十二指腸病変に対する膵温存十二指腸切除術

要旨
十二指腸主乳頭部周囲に発生した腫瘍に対しては，内視鏡的切除や経十二指腸的局所切除から膵頭十二指腸切除にいたるまで様々な術式が行われている．今回，機能温存手術としての膵温存十二指腸切除術の手術手技につき報告する．症例は70歳，男性．十二指腸主乳頭直上の腺腫に対して膵温存十二指腸切除術を施行した．術後，膵液・胆汁の外瘻化による一過性低Na血症，代謝性アシドーシスを経験したが，そのほかに合併症は認めず術後14日目に退院となった．退院後の食事摂取，栄養状態も非常に良好である．膵温存十二指腸切除術は複雑な術式ではあるが，機能温存手術として優れた術式と考えられる．
