膵管胃粘膜吻合

【ポイント】
◆膵頭十二指腸切除術の膵消化管再建法は，近年複数の大規模比較臨床試験にて，膵胃吻合が術後膵液瘻を含む術後短期成績において膵腸吻合を凌駕したと報告されている．
◆膵管胃粘膜吻合の手技のポイントは，膵切離は超音波凝固切開装置で行い分枝膵管のシーリングを行う，膵管胃粘膜吻合は8針以上行い内ステントを留置，膵管内の確実な減圧を行う，膵断端を胃粘膜ポケット内に陥入させ分枝膵管由来の膵液漏出を腹腔内に漏らさない，以上の3点である．
