【ポイント】
◆右側結腸周囲臓器との複雑な解剖学的関係を十分に理解し，腫瘍局在に応じた手技の定型化が重要である．
◆広範囲な結腸間膜授動に加え，豊富な血管バリエーションに対応した血管処理，リンパ節郭清が必要である．
◆超音波凝固切開装置は止血凝固能に優れ，右側結腸の剝離・授動に有用であるが，機器の原理を理解し適正に使用する．
◆完全なcomplete mesocolic excision（CME）のためには，十分なカウンタートラクションをかけた視野で超音波凝固切開装置を使用する．
