【ポイント】
◆診断：身体所見・諸検査所見から総合的に診断するとともに，患者の精神的・身体的苦痛や腫瘍学的治療の優先度も考慮して治療方針を決定することが重要である．
◆予防：最大の予防法は丁寧に吻合を行うことである．その前提のもとで，ドレーン挿入，経肛門的減圧ドレナージ，予防的ストーマ造設などの適応を慎重に判断する．
◆治療：基本はドレナージである．腹膜炎の範囲・程度やドレナージの有効性を速やかに診断し，経皮的ドレナージや手術を含む治療方針を決定する．
