敗血症性ショック

【ポイント】
◆敗血症の定義が25年ぶりに改訂され，敗血症は感染によって臓器障害をきたした状態と定義されることになった．
◆敗血症性ショックは，十分な輸液負荷にもかかわらず平均血圧≧65 mmHgを維持するのに血管収縮薬が必要，かつ血中乳酸値が2 mmol/L以上を呈する場合と定義される．
◆敗血症性ショックの初期治療では，抗菌薬投与前の培養検体の採取，1時間以内の抗菌薬投与，30 mL/kgの初期輸液投与，第一選択の昇圧薬としてノルアドレナリンの投与が重要である．
