はじめに
　腹腔鏡下胆囊摘出術は1990年前後に各国で臨床導入され，1990年代には胆囊結石症，胆囊良性疾患に対する標準術式として世界的に定着した．現在では，最も多く行われている腹腔鏡手術の一つである．日本内視鏡外科学会が2年ごとに行っているアンケート調査^1）によると，2013年の年間実施件数は27,809件，2000年と比較しても約36%増加している．その一方で，胆摘術に占める腹腔鏡下手術の割合は1995年以降一貫して80%前後で推移しており，腹腔鏡下胆囊摘出術の適応について一定のコンセンサスが得られ，すべての症例が腹腔鏡下に対処できないことも明らかになってきた．2016年度の診療報酬改訂で開腹胆摘術（K672：23,060点）の保険点数が腹腔鏡下胆囊摘出術（K672-2：21,500点）より高額になったことは，腹腔鏡下胆囊摘出術はそれぞれの医療機関の技量に応じて安全に施行できる症例に限定して実施することを推奨する社会的なメッセージでもあるとも考えられる．腹腔鏡下胆囊摘出術における最大の問題は，導入後25年以上経過した現在でも胆管損傷の頻度が開腹胆摘術と比較して明らかに高いことで，2012〜2013年の2年間だけでも304例（全症例の0.55%）がアンケート調査で報告されている^1）．
　Subtotal cholecystectomyは，高難度の胆摘症例，特にCalot三角周囲の炎症・線維化が強く胆囊管が安全に処理できない症例に対して，胆囊頸部を残し胆囊底部を切除して胆囊結石を摘出する術式である．腹腔鏡下胆囊摘出術の完遂困難例は，従来より開腹移行で対応されているが，術後疼痛の遷延，術後活動度の低下，在院日数の延長などが問題点として指摘されている^2,3）．腹腔鏡下胆囊摘出術におけるsubtotal cholecystectomy（laparoscopic subtotal cholecystectomy：LSC）は，それらの欠点を補い，開腹移行に代わる胆管損傷を回避しえる方法として行われている．しかし現時点では，実施の可否，適応，開腹移行との優劣について，一定の見解は得られていない．
