囊胞化した小腸GISTに対し超音波ガイド下ドレナージ後手術を行った1例

要旨
症例は60歳，男性．腹痛と発熱を主訴に当院を紹介され受診した．腹部CT検査で空腸から壁外性に発育する内部に気泡と液体貯留を有する腫瘍を認め，さらに腹腔内に充実性腫瘍の多発を認めた．囊胞変性した空腸腫瘍とその腹膜播種を疑ったが確定診断が得られなかったため，液体貯留に対し超音波ガイド下ドレナージを施行し精査を進めることとした．ドレナージ後約1か月の時点で炎症反応はほぼ正常化したが，腫瘍の確定診断には至らず，最終的に手術を行うこととなった．今回われわれは，囊胞変性した巨大小腸gastrointestinal stromal tumor（GIST）に対し超音波ガイド下ドレナージを行い，縮小させてから手術を行った1例を経験した．治療方針の決定に苦慮したため文献的考察を加え報告する．
