開腹手術versus腹腔鏡手術

【ポイント】
◆現状では腸管減圧前もしくは減圧不良の閉塞性大腸癌に対しては開腹手術が原則であり，減圧できた場合は腹腔鏡手術が開腹手術に加えオプションとなりうる可能性がある．
◆減圧できた閉塞性大腸癌例に対する腹腔鏡手術の短期成績は，開腹手術と比べ良好である．
◆現在進行中のRCT（COBRA試験，JCOG1107試験）の結果により，今後一定のコンセンサスが得られることが期待される．
