【ポイント】
◆幽門保存胃切除術はM領域の早期胃癌（cT1N0）を対象に行われる縮小手術である．最近は腹腔鏡下で行われることが多い．
◆噴門・幽門機能を温存する術式であり，術後stasisが発生しうるが，下痢・ダンピング発生抑制などの利点，ならびに術後栄養状態，予後良好と報告されている．
◆施行に際しては，術前の的確な腫瘍範囲の把握と，幽門下動静脈などの局所解剖に精通しておくことが肝要である．
