膿瘍形成性虫垂炎に対するlaparoscopic interval appendectomyの検討

要旨
【背景】膿瘍形成性虫垂炎に対する一期的手術は，拡大手術移行や合併症の発生率が高いため，当科では保存治療後に待機的に腹腔鏡下虫垂切除術を行うlaparoscopic interval appendectomy（LIA）を導入している．【目的】当科でのLIAの治療成績を報告し問題点を考察する．【対象と方法】2012年1月〜2016年3月に当院で加療を行った膿瘍形成性虫垂炎45例を対象とした．一期的手術を行った群と，初診時にLIAの方針となった群の治療成績を，カルテレビューによる後方視的解析を行い比較検討した．【結果】一期的手術は拡大手術になる可能性が高く，手術時間が長く，合併症率も高かった．総入院期間，総医療費は両群で差がなかった．【結語】LIAは有効な治療方針であると考えられる．
