膵頭神経叢の正体

　「膵頭神経叢の正体」というお題をいただいた．しかしながら，これは非常に難しい．解剖学用語には「膵神経叢」という用語はあるが，「膵頭神経叢」というのはないからである．よって，これは膵頭十二指腸切除の時に意識される「膵神経叢」の一部のことであると考えられる．本稿では，この一部である「膵頭神経叢」について考えることにする．
　膵臓の神経は，主に腹腔神経叢（腹腔動脈と上腸間膜動脈の起始部を取り巻く神経線維の集合）からくる交感神経と副交感神経である．もちろん内臓性知覚神経も含まれる．交感神経は大・小内臓神経，副交感神経は迷走神経に由来する．腹腔動脈と上腸間膜動脈は，膵頭部と膵体部の間に位置する膵頸部付近を上下から挟むように腹側に向かって走る．これらの動脈の通路は，膵頭部の膵後筋膜（Treitz膵後筋膜）と膵体・尾部の膵後筋膜（Toldt膵後筋膜）の間隙によってつくられる．神経もこの間隙を通って動脈とともに膵臓に達し，分布することになるのである．
