【ポイント】
◆Walled-off necrosisを含む膵周囲液体貯留に対する治療は，最近では超音波内視鏡ガイド下ドレナージが第一選択となることが多い．
◆可能であれば超音波内視鏡ガイド下ドレナージは，液体貯留が被包化される4週目以降に行う．
◆近年は初回ドレナージ時からネクロゼクトミーを行うよりも，低侵襲な治療から開始するstep-up approachが予後を改善するとされている．
