多発胃粘膜下腫瘍に対して2種類のアプローチを組み合わせ単孔式腹腔鏡手術を施行した3例

要旨
多発胃粘膜下腫瘍に対して2種類のアプローチで単孔式腹腔鏡手術を3例に施行した．症例1は，体上部前壁に20 mm，噴門部に5 mmの腫瘍を認めた．症例2は，体上部前壁に30 mm，噴門部に8 mmの腫瘍を認めた．症例3は，穹窿部の30 mmの腫瘍からの出血で内視鏡下止血術を施行され，噴門部に10 mmの腫瘍を認めた．全例で胃内手術を先行させ，噴門部の病変を切除後に腹腔内操作とし，他方の病変を漿膜側から局所切除した．病理検査ではいずれもGISTと平滑筋腫の診断であった．離れた部位にある胃粘膜下腫瘍に対して2種類の術式を組み合わせることにより，胃の切除・変形を最小限にとどめ，臓器温存性の高い手術が可能であった．
