リンパ節転移を伴った内腔発育型胃GISTにLECSを施行した1例

要旨
症例は78歳，女性．近医で胃粘膜下腫瘍を指摘され当院を紹介された．精査にて径20 mmの内腔発育型胃gastrointestinal stromal tumor（GIST）とリンパ節転移が診断された．手術は腹腔鏡内視鏡合同手術（laparoscopy endoscopy cooperative surgery：LECS）と転移リンパ節を含めたpick-up郭清を行った．術後病理組織診断で腫瘍は22×18×15 mm，紡錘形細胞が束状に増殖し，柵状配列や花筵配列を形成していた．免疫組織染色で胃GIST，リンパ節転移と診断された．リンパ節転移を認めておりclinically malignantと診断し，イマチニブの術後補助化学療法を施行した．術後1年で再発徴候は認めていない．リンパ節転移を伴う胃GISTに対するLECSと転移リンパ節pick-up郭清は，低侵襲手術として選択肢の一つと考えられた．リンパ節郭清の意義も含めた文献的考察を加えて報告する．
