【ポイント】
◆脾動脈根部が膵実質の背面深く走向している埋没型では，脾動脈根部処理が困難であることを認識しておく必要がある．
◆脾動脈と膵実質の間を交通する動脈をあらかじめ確認しておくことは，術中の出血予防に重要である．
◆脾動静脈を損傷しないために血管の腹側中央で切開・剥離を進めると，分枝の損傷リスクが低減する．
