完全腹腔鏡下手術における切離ラインの決定

【ポイント】
◆完全腹腔鏡下胃切除術においては様々な胃切離ライン決定法がある．いずれの方法においても，内視鏡での正確な病変範囲診断のもとに術前マーキングを行い，これを術中に認識して胃切離ラインを決定する．
◆幽門側2/3の切除範囲に含まれる病変においては，点墨によるマーキングなど簡便な胃切離ライン決定法で十分である．
◆高位病変では，術中内視鏡を用いて過不足ない胃切除を行うことで胃機能温存に努める．
