直腸癌肝転移と肝内胆管癌の同時性重複癌の1例

要旨
症例は64歳，男性．直腸腫瘍で近医より紹介となった．下部消化管内視鏡検査および腹部CT検査では，下部直腸の腫瘍および肝外側区域と肝S4区域に腫瘤影を認めRb直腸癌〔T3N0M1（肝）Stage Ⅳ〕と診断した．まず原発巣に対して腹腔鏡下腹会陰式直腸切断術を施行した．術後2か月のCTで肝転移は軽度増大するものの，新規病変を認めなかったため肝左葉切除術を施行した．術後の病理組織学的検査では肝S4区域の腫瘍は直腸癌肝転移であったが，肝外側区域の腫瘍は肝内胆管癌と診断された．術後は補助化学療法として双方の癌に共通するS-1を6か月間内服した．稀な直腸癌肝転移と肝内胆管癌の同時性重複癌の1例を経験したので報告する．
