【ポイント】
◆主に脂肪組織で構成される漿膜下層外層（SS-O）を内層（SS-I）から剝離するのが胆囊摘出術の基本手技である．
◆SS-OとSubvesical bile ductの関係を整理することで，術中胆道損傷の原因が理解しやすくなる．
◆炎症や虚血によって胆囊壁の層構造が失われると，手術難度が高くなる．
