【ポイント】
◆乳房外Paget病はアポクリン腺由来あるいは表皮内のmultipotential germ cell由来の癌である原発性と，直腸・肛門管癌などの上皮内進展（いわゆるPagetoid spread）による続発性（二次性）に分けられ，治療や予後が異なることから，両者を鑑別することが重要である．
◆原発性乳房外Paget病の治療は外科的切除が原則である．術前マッピング生検による病変範囲の確定後に完全切除を行うが，肛門周囲の病変については排便機能を考慮した慎重な術式選択が必要である．
◆手術困難例に対しては，化学療法や放射線療法以外に光線力学的治療やイミキモド外用療法などが治療オプションとなりうる．
