画像上自然消失が観察された多発肝細胞癌および腹膜播種の1例

要旨
78歳の男性．HCV抗体陽性．肝S8の3 cm大の肝細胞癌破裂に対し緊急transcatheter arterial embolization（TAE）を施行した．その後，肝部分切除は拒否された．6か月後，腹膜播種からの出血を発症し，開腹腫瘤切除術を施行し肝細胞癌腹膜播種を病理でも確認した．さらに3か月後に多発肝転移，多発腹膜播種再発を発症したが，さらなる治療は拒否されたため緩和医療に移行した．1年半後のCTにて肝腫瘤，腹膜播種腫瘤の完全消失を認めた．無治療下における，腹膜播種を伴う肝細胞癌の自然消失の報告は初である．
