大腸ステントの適応と薬物療法

【ポイント】
◆薬物療法（特に血管新生阻害薬）を行う患者に対するステント治療は，穿孔のリスクがあるため推奨されない．
◆RAS遺伝子変異の有無を確認し，全体の治療戦略を立案する前にステント留置を先行することは，重要なkey drugである血管新生阻害薬を使用できなくし，患者の予後を短縮する危険があることに留意すべきである．
◆外科的切除を前提とした術前の閉塞解除処置（bridge to surgery：BTS）としてのステント治療には複数の利点があるが，穿孔などが長期予後を悪化させる可能性も指摘されている．
